P Telephone:(209)468-3420 Fax:(209)464-0138 Web:www.sigov.org/ehd <br /> �Lhi <br /> Food Program Complaint Inspection Report <br /> Name of Facility: GLEASONS ICE CREAM Date: 03/30/2017 <br /> Address: 501 N SIERRA NEVADA ST, STOCKTON 95205 <br /> Owner/Operator: SOM, CHANSERY W Telephone: (209)941-8166 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00043157 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ORDERED A CINNAMON ROLL AT THE FACILITY AND FOUND A COCKROACH ON THE ROLL <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:Observed 2 dead cockroaches underneath the bakery case and about 15 dead cockroaches underneath <br /> a reach in freezer in the back area. <br /> Observed a few rodent droppings in the back area underneath equipment and above an old oven. <br /> Remove and implement pest control today. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #29 Toxic Substances Properly Identified/Stored/Used <br /> OBSERVATIONS:Facility is using boric acid powder to treat cockroaches. Discontinue use today and only use commercial <br /> or restaurant grade pest control. <br /> CALCODE DESCRIPTION:All poisonous substances, detergents, bleaches, and cleaning compounds shall be stored separate from food, <br /> utensils,packing material and food-contact surfaces.
