(Enviros) has prepared this Work Plan Addendum to the "Workplan for <br /> Additional Investigation and Product Removal" submitted by Fugro West, Inc. dated <br /> January 12, 1996. This addendum specifically responds to San Joaquin County Public <br /> Health Services (SJCPHS) comments from the meeting held between SJCPHS and Shell <br /> Oil Products Company (Shell) on February 2, 1996 and phone communications with <br /> Enviros for the referenced site(Figure 2). <br /> Attached to this addendum are the following: <br /> • A Site Map and Proposed Monitoring Well Locations, <br /> • A completed permit application for the installation of the three proposed wells <br /> • Well installation fees, and <br /> • A description of soil sampling and screening procedures, and selection of soils for <br /> chemical analysis. <br /> Well Application & Fees <br /> During the February 2, 1996 meeting, SJCPHS indicated that they had received a faxed <br /> copy of the Site Map that was not legible.
