WELL DESTRUCTION <br /> The overexcavation activities proposed in our July 16, 2004 Corrective Action <br /> Plan/Overexcavation Work Plan, included excavation in the vicinity of well MW-5 Therefore <br /> well MW-5 was properly destroyed on August 27, 2004 by aver-drilling the well casing with <br /> 12-inch diameter hollow-stem augers to a depth of 27 fbg The resulting borehole was backfilled <br /> with a cement-bentonite mixture to grade SJCEHD staff was onsite to witness the destruction <br /> and backfilling of well MW-5 A copy of the well destruction permit is included in Appendix A <br /> OVEREXCAVATION ACTIVITIES <br /> As part of an interim remedial effort, overexcavation activities were proposed in our July 16, <br /> 2004 Corrective Action PlanJOverexcavation Work Plan These activities were approved by <br /> SJCEHD staff in their July 20 , 2004 correspondence <br /> Overexcavation activities commenced on November 22, 2004 with the demolition of the southern <br /> dispenser canopy The actual soil excavation activities began on November 29, 2004 and were <br /> completed on December 2, 2004 after backfill and compaction was completed The total depth <br /> of excavation was approximately 14 fbg and an estimated total of 859 tons (-660 cubic yards) of <br /> • soil was removed and disposed of offsite <br /> 0783 4 <br />
