1 <br /> April 3, 1997 <br /> Ms. Mary Meays <br /> San Joaquin County Public Health Services <br /> Environmental Health Division <br /> Site Mitigation Unit <br /> P O Box 388 <br /> Stockton, California 95201-0388 <br /> RE: Monitoring Well Installation Report <br /> Shell Service Station <br /> 2575 Country Club Boulevard <br /> Stockton, California <br /> WIC 204-7524-3505 <br /> Dear Ms Meays <br /> This report documents drilling and monitoring well installation activities performed by <br /> Enviros, Inc (Enviros) on behalf of Shell Oil Products Company (Shell) at the above <br /> referenced site (Plates l and 2) The work included drilling four exploratory soil borings, <br /> converting three of the borings to ground water monitoring wells, performing associated <br /> soil and ground water sampling, and preparation of this report <br /> iThis work was performed in accordance with the "Workplan for Additional Investigation <br /> and Product Removal" submitted by Fugro West, Inc on January 12, 1996, Enviros' April <br /> 16, 1996 Work Plan Addendum, Enviros' October 10, 1996 letter, and San Joaquin <br /> County Public Health Services (SJCPHS) and Regional Water Quality Control Board <br /> guidelines <br /> 1.0 SITE DESCRIPTION <br /> The site is located on the northeastern corner of the intersection of Country Club Boulevard <br /> and Plymouth Road in Stockton, California The site is an operating service station with a <br /> site layout which includes a service station building, an underground fuel storage tank <br /> complex, four dispenser islands, and a waste oil tank <br /> On December 7, 1994 Shell notified SJCPHS of a suspected product loss from the regular- <br /> unleaded underground fuel storage tank (UST) at this site The total estimated release was <br /> approximately 8,500 gallons An estimated 822 gallons of product were recovered utilizing <br /> a vacuum truck <br /> Wells MW-1 through MW-5 and Well RW-1 were installed in mid-December 1994 <br /> Additionally, 14 exploratory soil borings were advanced at the site in late December 1994 <br /> using a cone penetrometer rig <br /> Soil vapor extraction was performed at the site from December 1994 through May 1995 <br /> 48 with an estimated 1,449 gallons of petroleum hydrocarbons removed during this period <br /> Cnvlroa, Inc 270 Perkins Street Sonoma Cai,torn,a Tel 707 1.935 4850 <br /> P 0 Box 259 95476 0259 Fax 701 i 935 6649 <br />
