No professional <br /> evaluation or interpretation was offered and PHS-EHD is unsure of your intentions or plans for this <br /> site. <br /> In order to avoid any further confusion in the future,you are hereby directed to provide <br /> professional evaluations in a conclusion and recommendation section in every report <br /> submitted to PHS-EHD. <br /> Enclosed please find a PHS-EHD policy regarding the collection and laboratory analyses for the fuel <br /> oxygenates in soil and groundwater samples. You are hereby directed to comply with this <br /> policy. As this site approaches the completion of its investigation and remediation phases, it is <br /> important to provide a complete analytical "picture" of the soil and groundwater plume. At least <br /> two consecutive complete groundwater sampling events that include all constituents that have <br /> previously been detected at this site should be included in the Request for Closure document so that <br /> PHS-EHD will be able to conduct the required evaluation. <br /> PHS-EHD records indicate that a sensitive receptor survey for this site has not been conducted. <br /> Please obtain the California Department of Water Resources (D)NIR) form(s) required to review <br /> their records of wells in the area of this site. PHS-EHD will sign the DWR request form as soon as <br /> it is forwarded to our office. <br /> In preparing a Request for Closure Report for this site,please follow the Appendix B,Tri-Regional <br /> Board Staff Recommendations for Preliminary Investigation and Evaluation of Underground tank <br /> Sites,page two,which lists the items to be submitted to PHS-EHD. An incomplete closure request <br /> report can delay review and processing of the documents to the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB) for their required notification and request for concurrence. <br /> A Division of San Joaquin County Health Care Services <br />
