(Stantec), on behalf of 7-Eleven, Inc. (7-Eleven), is <br /> submitting this email scope of work for the destruction of one onsite groundwater monitoring well (MW-2) at the 7-Eleven <br /> Store#14117 located at 2725 County Club Boulevard in Stockton, California. MW-2 is located in the center of the <br /> underground storage tank (UST) pit, between the two USTs (see attached figure). Due to a tank failure, 7-Eleven has <br /> scheduled the USTs at this location to be removed. As such, Stantec, on behalf of 7-Eleven, is requesting approval of the <br /> following scope of work to destroy groundwater monitoring well MW-2. Records show that MW-2 was installed on May <br /> 13, 1999 by advancing a two-inch poly-vinyl-chloride (PVC) pipe through an existing 10-inch PVC conductor casing that <br /> extends to 13 feet below ground surface (bgs) (Attached MW-2 Log). <br /> Please note that UST removal activities are scheduled to begin on June 17, 2013. Stantec would prefer to properly <br /> destroy monitoring well MW-2 prior to the UST removal. <br /> SCOPE OF WORK <br /> Permitting/Utility Clearance— Upon approval of this scope of work, Stantec will secure expedited well destruction <br /> permits with the San Joaquin County Environmental Health Department(SJCEHD). Prior to conducting subsurface work <br /> at the site, Underground Service Alert(USA) and a private utility locator service will be contacted to delineate subsurface <br /> facilities. <br /> Health and Safety -Stantec will modify the existing site-specific Health and Safety Plan for the scope of work as required <br /> by the Occupational Health and Safety Administration (OSHA) Standard "Hazardous Waste Operations and Emergency <br /> Response"guidelines (29 CFR 1910.120). The document will be reviewed and signed by Stantec personnel and <br /> subcontractors performing work at the site. <br /> Well Destruction—Due to the location of MW-2 between the two USTs, Stantec is requesting a variance from the <br /> SJCEHD requirement to overdrill the well to total depth. As suggested by Mr. Newel Henderson of the SJCEHD, it may be <br /> acceptable to destroy the well by pressure grouting from the bottom of the well to 13 feet bgs. The groundwater <br /> monitoring well PVC casing will be pulled out; the remaining 10-inch PVC conductor casing and the well box will be <br /> removed during the UST removal activities. <br /> WASTE DISPOSAL <br /> Debris generated during well destruction will be temporarily stored at the 7-Eleven site in Department of Transportation- <br /> approved 55-gallon drums, pending analytical profiling and offsite disposal. <br /> REPORTING <br /> Upon receipt by Stantec of waste disposal documentation, a report documenting the well destruction activities will be <br /> submitted to the SJCEHD. <br /> i <br />
