A site map shall contain a north orientation, loading areas, internal <br /> roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging <br /> areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is <br /> not applicable for your facility then list it on the map and label as"NA". Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> This is a repeat violation, Class ll. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually for 2018 and 2019. The hazardous materials inventory shall be submitted by January 15 of <br /> each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions for <br /> ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented <br /> electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide <br /> employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
