Avenue, Lathrop, San <br /> Joaquin County, California <br /> Dear Ms. Lagorio:- <br /> On March 9, 2004 a representative of Condor Earth Technologies, Inc. (Condor) was on site at the <br /> Pilkington North_America (PNA) facility located at_500 East Louise Avenue, Lathrop, San Joaquin <br /> County, California to observe the destruction of groundwater monitoring wells MW-17 and MW-18. Site <br /> Vicinity and Plan maps are attached (Figures 1 and 2). The well destructions were performed by V & W <br /> Drilling, Inc. (V&W) of Isleton, California (C57 License#720904). Destruction of the two monitoring <br /> wells was requested by Ms. Margaret Lagorio of the San Joaquin County Environmental Health <br /> Department (SJCEHD) in a letter to Mr. Roger Bishop of PNA.dated November 10, 2003, following <br /> concurrence from the Central Valley Regional Water Quality Control Board(CVRWQCB) that no further <br /> investigation or remediation is required at the site. Condor has prepared this reportto document site <br /> activities which satisfy "No Further Action"requirements of the SJCEDH and CVRWQCB necessary for <br /> site closure. <br /> A permit to destroy the monitoring wells was obtained from the SJCEHD by Condor prior to the <br /> destruction of the two wells. A copy of the well destruction permit issued by SJCEHD is attached.. Ms. <br /> Margaret Lagorio of the SJCEHD was on site to observe a portion of the monitoring well destruction <br /> activities. <br /> According to available welt logs, MW-17 and MW-18 were installed with 8-inch O.D. hollow stem <br /> augers to depths of 19.5 and 19:0 feet respectively, and were constructed with two-inch, schedule 40 PVC <br /> piping with approximately fifteen feet.of 0.030 slotted screen and #3 sand pack backfill. On March 9, <br /> 2004 the total depths of monitoring wells MW-17 and MW-18 were measured at 17.72 and 16.81 feet <br /> respectively. The static water levels in the two monitoring wells on March 9, 2004 were measured at 8.61 <br /> and 6.83 feet respectively,below tops of well casing. <br /> The two monitoring wells were destroyed by inserting a tremie pipe into the casing and filling the wells <br /> from the bottom up with a grout slurry to displace the water in each well. After the casing of each well <br /> was filled with the grout slurry, a pressure adaptor was fitted to the top of the casing and a minimum of 35 <br />
