Weber Avenue, 3`d Floor <br /> Stockton, CA 95202 <br /> Re: Work Plan for Monitor Well Closure, Pilkington North America, 500 East Louise <br /> Avenue,Lathrop, San Joaquin County, California <br /> Dear Mr. Knoll: <br /> q <br /> This Work Plan describes the closure of two monitoring wells, MW4 and MW-18, at the above <br /> referenced site. Condor Earth Technologies, Inc. (Condor) prepared this work plan at the request <br /> of Mr. Roger Bishop with Pilkington North America (PNA). A Well Permit Application Form <br /> with application fees for the destruction of the two monitoring wells is attached to this Work <br /> Plan. <br /> The two monitoring wells at the site will be properly destroyed in accordance with the procedures <br /> described in this Work Plan as part of the site closure process. The well destructions will be <br /> performed by V&W Drilling, Inc. (C57 License #720904). Condor personnel under the direct <br /> supervision of a California Registered Geologist will oversee the well destructions. Upon receipt <br /> of the permit, Condor will schedule and coordinate the fieldwork. The San Joaquin County <br /> Environmental Health Department (SJCEHD) will be notified at least 48 hours before initiation <br /> of fieldwork. At least 48 hours prior to initiating the fieldwork, the monitor well locations will be <br /> marked with white paint and Underground Service Alert (USA) will be notified to locate and <br /> identify underground utilities in the vicinity of the work areas. In addition, a tailgate safety <br /> meeting will be held and a site-specific Health and Safety Plan will be discussed with, and <br /> presented to, all field personnel for their signatures. <br /> Monitor Well Destruction <br /> Condor will supervise the destruction of monitor wells s MW-17 and MW-18, which were installed <br /> by Clark Well and Equipment under the supervision of Woodward-Clyde Consultants in 1986. <br /> Based on available information, both monitoring well borings were advanced with 8-inch O.D. <br /> hollow stem augers to a depth of approximately 20 feet below ground surface (bgs). The <br /> monitoring wells were constructed with two-inch, schedule 40 PVC piping with approximately <br /> fifteen feet of 0.030 slotted screen and#3 sand pack backfill. <br /> The monitor wells will be destroyed by removing the traffic box and pressure grouting from the <br /> bottom up, by inserting a tremie pipe into the well bore and displacing the well water with a <br />
