(Condor) for <br /> Libbey Owens Ford Co. (LOF)during the closure of four monitor wells associated with the former above <br /> ground fuel oil storage tank (AST). The monitor wells were closed on May 5, 1995, at the above <br /> referenced facility. This report describes the field procedures used to close the monitor wells and <br /> summarizes the field observations made while the work was performed. Prior to performance of the field <br /> work, a monitor well closure permit was obtained from the San Joaquin County Environmental Health <br /> Department, Environmental Health Division (County). A copy of the approved permit application is <br /> attached to this report. <br /> Monitor Well Closure <br /> Monitor Wells MW-3, MWA, MW 92-1 and MW-2 were over-drilled and grouted by Spectrum <br /> Exploration, Inc. (Spectrum) of Stockton, California (C57 License #512268), under the supervision of <br /> Condor. Each monitor well was drilled out and the resulting borehole completely backfilled with a <br /> cement and bentonite mixture (neat cement). <br /> Monitor Wells MW-3 and MW-4 were constructed with ten (10) feet of well screen and six (6) feet of <br /> blank casing; Monitor Wells MW92-1 and MW92-2 were constructed with 15 feet of well screen and five <br /> (5) feet of blank casing. At each monitor well, the monument box, well cap and surface grout was <br /> removed by Spectrum and disposed of by LOF. The 4-inch PVC well casing was then pulled out of the <br /> well, cut up and disposed of in the same manner. The monitor well casing was removed intact from <br /> Monitor Wells MW92-1, MW92-2 and MW-3. During the closure of Monitor Well MW-4, the well <br /> casing broke into two pieces during the attempt to remove it. Approximately six (6) feet of the casing <br /> was removed and approximately ten (10) feet of the casing remained in the borehole (including all of the <br /> ENVIRONMENTAL GEOLOGICAL ENGINEERING GEOTECHNICAL <br />
