Pagel of 3
Lori Duncan [EH]
From: Adrienne Ellsaesser[EH]
Sent: Friday, March 11, 2011 9:44 AM
To: Bill Cook
Cc: Lori Duncan [EH]
Subject: FW: 500 E Louise
Attachments: WELL BORING PERMIT Final 7-28-10.pdf; Master File Record Green and Pink Unit IV
final.pdf
Bill,
My 2/23/11 e-mail comments regarding the MFR was based on the 2965 water quality program file. I was
not aware that your submittal would be for a Regional Board oversight site mitigation program, which
we refer to as a 2960 program.
Lori just explained to me there are two files for this site . One relates to the ponds/discharge permit
issues, one relates to the diesel spill issues. The work presented to Lori appears to be related to the diesel
spill issue, which is the 2960 file, and we do not have a MFR in that file.
We need an MFR form to be issued for this site under the 2960 program to process your permits. I
appologize for this inconvience and initial misunderstanding on my part.
I have attached the form for your use.
Also , the permit application that was submitted does not have a signature on it. How do you want to
resubmit the permit? Resend a new permit that has been signed by the approprate person ( one is
attached for your use) , send that person to our office to sign it,we scan and send it back to you for the
signature?
Once we have the completed MFR form and the signed permit, Lori can begain to process and review your
submittal.
Contact me if you any further questions about the permitting process.
Thank you,
Adrienne
Adrienne Ellsoesser, REHS
Acting Program Coordinator
Small Public Water System & Local Oversight Programs
San Joaquin County Environmental Health Department
600 East Main Street
Stockton CA 95202
Desk: (209)468-0343 Fax: (209) 468-0341
cellsaesser@sjcehd.com
3/11/2011
