It was provided to Mr. Pawar. <br /> November 29, 2011 - Benjamin Escotto and Robert McClellon performed the re- <br /> inspection (see report 11-122665 8). <br /> February 14, 2013 - Harprit Mattu wrote in the activity log of the Tires 4 Less file that <br /> the hauling vehicles are parked at Low Price New Used Tires (TPID # 1562878). Mr. <br /> Pawar owner of both facilities manages and keeps most of the documents for Tires 4 Less <br /> at Low Price New Used Tires. Harprit Mattu recommended performing the waste tire <br /> survey for both facilities on the same day. <br /> April 29, 2013 - Harprit Mattu performed a waste tire survey for TPID# 1047282 and <br /> noted that a Tires 4 Less CTL was onsite but was not found during a search of WTMS <br /> (see report I1-1177534). <br /> September 6, 2013 - Alfonso Arambula performed a waste tire survey for TPID <br /> #1757341, as part of a complaint and noted that Tires 4 Less performed the removal of <br /> waste tires associated with a clean up. Tire 4 Less provided two CTLs to the generator <br /> (see report I1-1210521). The CTLs for the delivery of those tires are not found in <br /> WTMS. <br /> November 26, 2013 - Harprit Mattu performed a waste tire inspection at Manteca Ford <br /> TPID # 1047282 and noted a CTL fiom Tires 4 Less was onsite for review (see report I1- <br /> 1196673). The CTLs for the delivery of those tires are not found in WTMS. <br /> December 3,2013 -Alfonso Arambula began the waste tire survey(I1-1196619) for <br /> Tires 4 Less at 260 Button Ave. Alfonso Arambula was informed by an employee that <br /> all manifests are kept at 108 Industrial Park Dr. Alfonso Arambula performed a tire <br /> count and asked how junk tires are disposed of. Mike, an employee, stated that waste <br /> tires are normally stored in a boxed van Ford 450. The Ford vehicle then transports the <br /> waste tires to Low Price Tires for consolidation. Mike also stated Shamrock(TPID # <br /> 1284842) occasionally performs waste tire pickups. Alfonso Arambula then traveled to <br /> 108 Industrial Park Dr to perform the manifest review for Tires 4 Less. <br /> December 9,2013- The following hauler manifest review on the waste tire management <br /> system (wtms) for Tires 4 Less (TPID# 1661994) shows discrepancies. In 2011 a CTL <br /> was submitted to Calrecycle showing a pickup and delivery of 309 tires. In 2012 eighteen <br /> total manifests were submitted to Calrecycle. Seventeen manifests reported to Calrecycle <br /> showed pickups performed for a total of 1785 tires. One CTL reported to Calrecycle <br /> showed a delivery for 54 tires. In 2013 zero manifests were reported to Calrecycle as of <br /> December 3, 2013. <br /> 4 <br />
