Jose Rios and Howard Sweet: <br /> The San Joaquin County Environmental Health Department (EHD) has received no comments <br /> or concerns from interested individuals and agencies regarding the pending closure of your site <br /> during the sixty-day public participation period which ended 22 November 2013. Therefore, the <br /> EHD directs that you proceed with the well destructions as approved by the EHD by letter dated <br /> 24 June 2013. Please submit an EHD Well and Boring Permit Application form along with a <br /> Master File Record form, Licensed Contractor's Declaration form and appropriate fees. The <br /> permit fee for the destruction of each well is $125, along with a $375 Inspection Activity fee per <br /> parcel. A review of the site records indicate that seven monitoring wells and two dual-nested <br /> remediation wells (SPV-1 and SPV-2) are installed at your site. A fee of $1,500.00 (9 x $125 + <br /> $375) will be required for the well destruction permit. (To determine the fee for nested wells, <br /> each nested well is considered as one well for permitting and fee purposes.) Once the wells <br /> have been properly destroyed and the well destruction report, which must include the manifests <br /> signed by facilities that receive the waste generated during the well destructions, has been <br /> submitted to the EHD, and all fees have been paid to the EHD, this agency will issue a No <br /> Further Action Required letter for the above-referenced site. <br /> If you have any questions or concerns, please contact Vicki McCartney at (209) 468-9852 or by <br /> e-mail at vmccartney@sjcehd.com. <br /> Sincerely, <br /> Ile .. <br /> Victoria L.
