SANJ O A O U I N Environmental Health Department <br /> CUUN I Y- <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> AMAZON.COM SERVICES, INC-SMF3 3923 S B ST; STOCKTON June 27, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> The Hazardous Materials Inventory forms for bleach solutions, amines, aerosols, hydrogen peroxide solutions, lead <br /> acid batteries, liquid pesticides, Non-RCRA hazardous waste liquids, used oil, organic toxic liquid, petroleum <br /> distillates, waste corrosive liquid(acidic), and waste corrosives liquid (basic)were not accurate. The amount of <br /> bleach solutions, amines, hydrogen peroxide solutions, lead acid batteries, liquid pesticides, Non-RCRA hazardous <br /> waste liquids, used oil, organic toxic liquid, petroleum distillates, waste corrosive liquid (acidic), and waste <br /> corrosives liquid(basic)was wrongly identified in"pounds"vs. "gallons"; the amount of aerosols was identified in <br /> "pounds"vs. "gallons" or"cubic foot". This information must be accurate and submitted as part of the business <br /> plan. Immediately log into the California Environmental Reporting System (CERS)at hftp://cers.calepa.ca.gov/, <br /> enter the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map did not show/address loading areas, internal roads, and hazardous material handling and storage areas <br /> for the diesel fuel. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm <br /> and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a Class 11 violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
