The site map uploaded to CERS does not reflect the changes made to the copy of the site map <br /> submitted via FedEx and email. <br /> 2. The hard copy map submittal does not show the location of diesel fuel and does not clearly show <br /> the storage location of the "hazardous materials cage". <br /> 3. The site map uploaded to CERS does not show/address loading areas, internal roads, adjacent <br /> streets, and hazardous material handling and storage areas for the diesel fuel. A site map shall <br /> contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. if a site map element is not applicable for <br /> Your facility then list it on the map and label as "NA". <br /> • Violation#11—The employee training records are not complete. <br /> - Some of the employee training records do not identify the training topic/element. <br /> - None of the employee training records identify the name of the employees trained. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
