ATC Associates Inc. <br /> Quality Assurance/Quality Control Plan <br /> Revised February 1995 <br /> The chain-of-custody record will contain the following information: <br /> ✓ site name <br /> ✓ signature of collector <br /> ✓ date and time of collection <br /> ✓ sample identification number(s) <br /> ✓ number of containers in sample set <br /> ✓ description of sample and container(s) <br /> ✓ name and signature of persons, and the companies or agencies they represent, <br /> who are involved in the chain of possession <br /> ✓ inclusive dates and times of possession <br /> ✓ requested analysis for each sample <br /> 5.3 Delivery of Samples to Laboratory <br /> Samples will be delivered to the laboratory on a daily basis. Samples will be refrigerated to <br /> approximately 4°C for shipping. Shipping containers will be sealed with security tape to assure <br /> sample integrity during shipping. Delivered samples will be accompanied by a chain-of-custody <br /> record. The laboratory shall note on the chain-of-custody that samples were properly <br /> preserved and security tape was intact upon arrival. <br /> 6.0 SAMPLING AND DRILLING EQUIPMENT DECONTAMINATION <br /> All equipment used for drilling and sampling will be decontaminated prior to use. All equipment used <br /> for collection of more than one sample, such as bailers and spoons, will be decontaminated between <br /> each use to prevent cross contamination between samples. Drilling equipment will be decontaminated <br /> prior to commencing drilling at each hole.
