Based on the review of your submittal, <br /> we have determined that the RWD is incomplete, as discussed below. <br /> 1. The City proposes to convert Land Application Area #3 (LAA3) into percolation ponds. <br /> However, the City does not own the land application area. During the 29 April 2014 site <br /> visit meeting, City staff stated that the ownership of LAA3 will be transferred to the City <br /> after completion of percolation pond construction. The City needs to submit an <br /> agreement signed by the land owner for the future ownership transfer. Otherwise the <br /> LAA3 land owner will be the co-discharger and must submit a Form 200. If the transfer <br /> is not completed before adoption of the revised Waste Discharge Requirements, the <br /> Board can adopt an ownership change order after the land transfer is completed. <br /> 2. Monitoring well KMW-4 is located on the proposed percolation pond site in LAA3. <br /> In addition, four piezometers were installed in LAA3 during the pilot test in spring 2013. <br /> Well KMW-4 and all piezometers should be appropriately abandoned before LAA3 is <br /> converted to percolation ponds. At least three monitoring wells should be installed <br /> outside of the LAA3 area. Please submit a Monitoring Well Abandonment and <br /> Installation Workplan as an addendum to RWD. <br /> 3. Please submit copies of the Draft and Final EIR and each addendum to the Final EIR <br /> that concerns the WWTF in any way. If the City approved some type of project in <br /> connection with any of the addenda, please provide documents that link each addendum <br /> to the action(s) it was associated with (e.g., minutes from the City council meetings, <br /> Notices of Action, etc.). <br /> KARL E.
