ADDENDUM TO THE APPLICATION FOR PERMIT FOR 0 <br /> UNDERGROUND TANK REMOVAL AT SANTA FE RAILWAY <br /> COMPANY'S STOCKTON REFUELING FACILITY <br /> Copies of the rinsate manifest, tank destruction certificates, underground tank disposition <br /> tracking record, backfill compaction report from certified soils lab and soil sample analysis <br /> reports will be forwarded to San Joaquin County Public Health Services (SJPHS) as part of <br /> our Final Closure Report- <br /> All <br /> eportAll product piping associated with underground tanks will be located and removed Pipe will <br /> be rinsed out and cleaned before shipping to the disposal site. A soil sample will be taken <br /> every 20 feet along the entire length of underground product pipelines. <br /> • The nnsate will be transported to. <br /> Recylcetron Oil,Inc. <br /> 1331 North Highway 33 <br /> Patterson, Califomia 95363 <br /> (800) 874-4444 <br /> The tanks and all associated product piping will be transported to: 0 <br /> Triangle,Inc. <br /> 3525 52nd Avenue <br /> Sacramento,California 95823 <br /> (916) 421-1990 <br /> • Soil samples will be taken 2 to 3 feet below the bottom of the tank cavities with a backhoe <br /> bucket An Environmental Solutions,Inc. geologist will then utilize standard stainless <br /> steel tubes or brass cylinders to take the required soil samples from the backhoe bucket. <br /> The tubes will be capped,sealed,placed into a cooler packed with ice and transported under <br /> Chain-Of-Custody to a state-certified laboratory. <br /> • After the application for tank removal has been approved by SJPHS we will apply to the City <br /> of Stockton Fie Department for a fire permit. This permit,as well as the tank removal permit, <br /> will be kept at the job site for inspection throughout the duration of the work <br /> • The Health and Safety Plan will be reviewed,signed,and dated by all personnel working on <br /> this project at the morning tailgate meeting held at the job site. . <br /> • Underground Services Alert(1-800-642-2444)will be notified at least two working days and <br /> no more than two weeks before the start of the work. <br />
