The well survey information is incomplete. Domestic wells exist <br /> within a 2000 foot radius of the above referenced site that are not shown on the well <br /> survey map. Every well located within a 2000 foot radius of the site should be shown on <br /> the well survey map. All wells should be clearly correlated to address locations. Any <br /> well logs or construction information should be submitted. <br /> Feasibility tests should be conducted in order to evaluate at least two different <br /> remediation alternatives for soil and water clean up at this site. QM should continue at <br /> this site until otherwise directed by PHS/EHD. Both on-site domestic wells should be <br /> included in the QM schedule and sampled as recommended above.
