Submit an accurate scaled map showing the borders:'of the proposed excavation taking into account the <br /> exact location of the fiber-optic line that the Work Plan states exists underground at the site. If the <br /> location of underground utilities at the site impede the projected amounts of contaminated soil from <br /> being removed and prevent post-treatment goals for contaminant source reduction from being met,a <br /> pilot test for AS/SVE must be conducted. <br /> 12. Soil confirmation samples must be collected midway through the one-year attenuation time period to <br /> monitor attenuation progress and again at the one-year mark following the end of the attenuation <br /> monitoring period. If soil sample analysis does not confirm attenuation, conduct a pilot test and <br /> employ AS/SVE. <br /> 13. In the event that any residential drinking water well evidences contaminant levels which exceed the <br /> maximum allowable contaminant levels during the attenuation monitoring period you must notify the <br /> 1 <br /> property owner of the well(s) and provide potable drinking water. Remedial a tematives must be <br /> r <br /> evaluated if any drinking water well becomes impacted with contaminants and a more aggressive <br /> remedial method must be employed at the site within 90 days. A contingency plan outlining <br /> procedures to be taken should residential drinking water wells be contaminated must be submitted to <br /> PHSIEHD prior to beginning the source reduction work. <br /> 14. Because of the close proximity of potential receptors at this site and due to the approach of the rainy <br /> season, which could further delay remedial work,the excavation work must be completed no later than <br /> December 15,2000. <br /> The sooner the above adequate and necessary elements are provided to PHSIEHD for the Work Plan the <br /> sooner corrective action can occur at this site and public health and safety protected.
