The Hazardous Materials Inventory form for carbon dioxide was not accurate. The maximum amount of carbon dioxide is wrongly identified as 300 cubic feet vs. 1,573 cubic feet. This information must be accurate and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. The map did not show/address loading areas, internal roads, storm and sewer drains, access and exit points, emergency shutoff, and some of the codes/symbols used on the map are not included in the legend. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS) annually by January 15 for 2018 and 2019. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. During the inspection 1,573 cubic feet (180 pounds) of carbon dioxide was found and the updated information has not been submitted. Within 30 days of one the following events, business shall electronically update the information submitted to the California Environmental Reporting System (CERS): -A 100 percent or more increase in the quantity of a previously disclosed material. -Any handling of a previously undisclosed hazardous materials -A change of business address, business ownership, or business name.
