The following is an itemized list of aboveground petroleum storage act violations that <br /> have not been addressed for DAMERON HOSPITAL as of July 08, 2019. <br /> Open violations from October 25,2018 inspection <br /> Violation#302-Failed to review Plan once every five years and/or implement any resulting amendments. <br /> The Spill Prevention, Control, and Countermeasure(SPCC) Plan was last reviewed on March 31,2011. A review <br /> and evaluation of the SPCC Plan must be conducted at least once every 5 years. As a result of this review and <br /> evaluation,the SPCC Plan must be amended within 6 months of review, and recertified by a Professional Engineer <br /> if any technical amendments were made. Immediately conduct a review of the facility SPCC Plan and make any <br /> necessary amendments. <br /> Violation#601 -Failure of Plan to fulfill basic requirements including a cross-referencing section. <br /> The Spill Prevention, Control, and Countermeasure(SPCC) Plan does not follow the order or requirements in 40 <br /> CFR Part 112, and a cross-reference was not provided. If you do not follow the sequence specified in 40 CFR 112 <br /> for the Plan, you must prepare an equivalent Plan and supplement it with a section cross-referencing the location of <br /> requirements in 40 CFR 112 and the equivalent Plan. Immediately amend the SPCC Plan to include a <br /> cross-reference or to follow the required sequence. <br /> Violation#603-Failed to adequately describe the physical layout of the facility in the Plan. <br /> The Underground Storage Tank(UST) is not marked as"exempt'on the facility diagram. The Spill Prevention, <br /> Control, and Countermeasure(SPCC) Plan shall include a facility diagram which must mark the location and <br /> contents of each fixed storage container and the storage area where mobile or portable containers are located. It <br /> must identify the location of and mark as"exempt'underground tanks. It must also include all transfer stations and <br /> connecting pipes, including intra-facility gathering lines. Immediately update the facility diagram to include all of the <br /> required information. Submit a legible copy of the updated facility diagram to the EHD for review. <br /> Violation#611 -Plan failed to include a description of potential equipment failure or provide equivalence. <br /> Potential equipment failure and the resulting spill for the 275 gallon day tank, 150 gallon day tanks and the 100 <br /> gallon day tank were not addressed in the Spill Prevention, Control, and Countermeasure(SPCC)Plan. The SPCC <br /> Plan must include a prediction of the direction, rate of flow, and total quantity of oil which could be discharged from <br /> the facility as a result of major equipment failure. Immediately amend the SPCC Plan to include this information and <br /> submit a copy of the revision to the EHD, or provide equivalence as allowed by 40 CFR 112.7(a)(2). <br /> Violation#628-Failure to include in the Plan discussion of conformance with Federal and State <br /> requirements. <br /> The SPCC plan did not include a discussion on conformance with Federal and State requirements. In addition to the <br /> minimal prevention standards listed under this section, include in your Plan a complete discussion of conformance <br /> with the applicable requirements and other effective discharge prevention and containment procedures listed in this <br /> part or any applicable more stringent State rules, regulations, and guidelines. <br /> Amend th SPCC plan to meet the requirements as described above. <br /> Violation#706-Failed to provide and maintain adequate secondary containment. <br /> The 100 gallon and 250 gallon day use tanks were observed with insufficient secondary containment. The tanks <br /> appeared to be single walled tanks and secondary containment was not evident for the tanks. All bulk storage tanks <br /> must be provided with a secondary means of containment for the entire capacity of the tank and sufficient freeboard <br /> to contain precipitation. Immediately provide sufficient secondary containment for this and all other tanks at this <br /> facility. <br /> Page 1 of 2 <br />
