Grantline <br /> Tracy, CA 95736 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) <br /> has not received a quarterly monitoring report for these sites since Second Quarter, <br /> 1995. Missing from both sites are reports for Third and Fourth Quarter, 1995, and First <br /> Quarter 1996. Any reports already compiled should be submitted immediately. <br /> Pursuant to Underground Storage Tank Regulations (California Code of Regulations, <br /> Title 23, Chapter 16, Section 2652 (d)), a quarterly monitoring report is to be submitted <br /> every three months until PHS-EHD deems the investigation and remediation completed. <br /> Sites are required to be in compliance with all regulations pertaining to underground <br /> tank regulations and directives issued by PHS-EHD. Failure to comply will jeopardize <br /> your eligibility to become or remain eligible to receive reimbursement under the <br /> Cleanup Fund and subject you to formal actions. <br /> You are hereby directed to monitor, sample, and analyze groundwater and report <br /> the findings in a quarterly report due at PHS-EHD prior to the next sampling <br /> event. Your reports are due by the fifteenth of the month following the quarter's <br /> end. <br /> This warning is to be considered your official notice as required by the State Water <br /> Resources Control Board, Underground Storage Tank Cleanup Fund that your site is in <br /> non-compliance and subject to re-evaluation in order to obtain or remain eligible to <br /> receive financial reimbursement. <br /> A Division of San Joaquin County Health Care Services <br />
