The plan is to identify responsible persons, complete with twenty- <br /> four hour emergency phone numbers, and the name, address, and a map to an acceptable <br /> emergency facility capable of handling injured personnel from the site. Submittal of this <br /> plan is due by December 20, 1993. <br /> A completed boring permit application and a $89 application fee is to be submitted to PHS- <br /> EHD by December 20, 1993. Include with your submittal a time schedule that provides dates <br /> when borings under this current phase will begin, and when a report of findings will be <br /> submitted to PHS-EHD. <br /> Complete quarterly monitoring reports as per Section 2652(d) of the Underground Storage <br /> Tank Regulations (Calif. T-23, Chap 16) shall be submitted to PHS-EHD no later than the <br /> 15th of the month following the quarter's end. This due date is for January, April, July, and <br /> October and shall include at least, ail of the items listed herewith: <br /> -status of investigation and cleanup activities including the results of all investigations <br /> to date; <br /> -proposed, continuing, or next phase of investigation; <br /> -method of cleanup propbsed or implemented, schedules for drill dates and report <br /> submittal, and the completion of investigation and/or remediation; <br /> A Division of San Joaquin County Halth f-2re Services <br />
