(L&P)has prepared this brief report of <br /> the destruction of five groundwater monitoring wells associated with the subject site, located at 2360 <br /> East Street, Tracy, California. The work was conducted pursuant to the Work Plan for Monitoring <br /> Well Destruction dated January 15,2010 under permits issued by San Joaquin County Environmental <br /> Health Department (SJC/EHD) and the City of Tracy. Figure 1 shows the site location. Figure 2 <br /> shows the locations of site monitoring wells. Attachment 1 includes copies of the well destruction <br /> permits issued by SJC/EHD and the encroachment permit issued by the City of Tracy. <br /> L&P scheduled the field work with Fisch Environmental (C57-683865) of Hydesville, California to <br /> provide well destruction services, updated the Site Safety Plan to include well destruction services, <br /> marked the well locations with white paint, and notified Underground Service Alert (USA) to mark <br /> underground utilities prior to drilling. L&P also coordinated the drilling date with SJC/EHD. <br /> On March 18, 2010, John Lane (PG 6795) oversaw Fisch Environmental destroy four on-site wells <br /> (MW-1, MW-2, MW-3, MW-4) and one off-site well (MW-5). Mr. Michael Infiuna, Senior <br /> Registered Environmental Health Specialist for SJC/EHD, was present on site to observe, inspect, <br /> and approve well destruction activities. The wells were destroyed pursuant to California Water <br /> Standards Bulletins 74-81 and 74-90 and the San Joaquin County Well Standards. <br /> Prior to well destruction, L&P removed all down-hole equipment from each well (dedicated tubing <br /> and Waterra inertial foot valves) and monitored the depth to water and depth to bottom in each well <br /> to insure that no obstructions were present. Fisch Environmental destroyed monitoring wells MW-1 <br /> through MW-3 (4-inch diameter wells) by removing the well vaults, over-drilling the annular seal <br /> with 10-inch hollow stem augers to a sufficient depth to loosen the PVC well casing (typically a <br /> depth of approximately 3 feet), and removing the contents of the well. Prior to removing the well <br />
