You are hereby directed to do an environmental investigation to determine the extent of soil and groundwater contamination as a result of the catastrophic release of petroleum products from the failed precision test verbally reported on January 11, 1991 by Lois Frantz of WaterWork. ' As per California Health and Safety Code, Div. 20, Chapter 6 . 7 , Article 25295, .a full written report shall be transmitted to local authorities within five (5) working days. Failure to provide a written report to this office now places you liable for civil penalties under Article 25299 under Chapter6 .7 . JI A work plan that includes methods of determining the lateral and vertical extent of contamination along with proposed remediation of soil and or' groundwater that may be impacted is due in our office within. thirty (30) days prior to any site work. Fees for review activities and inspections associated with contaminated site assessment activities will not be billed by this office. The State Water Resource Control Board (SWRCB) will invoice you directly for activities performed under the auspices of the Local UST Oversight Program. The goal of the site evaluation performed by Public Health Services Environmental. � Health Division is to certify closures of underground tank leakage sites. Closure certification occurs only after mitigation activities are completed. A Letter of Acknowledgment is enclosed for your completion and return. Your acknowledgment of the placement of your site into _ -.the Local UST Oversight Program as stated above is respectfully requested.
