You are <br /> hereby directed to do an environmental investigation to determine <br /> the extent of soil and groundwater contamination as a result of <br /> the catastrophic release of petroleum products from the failed <br /> precision test verbally reported on January 11, 1991 by Lois <br /> Frantz of WaterWork. ' As per California Health and Safety Code, <br /> Div. 20, Chapter 6 . 7 , Article 25295, .a full written report shall <br /> be transmitted to local authorities within five (5) working days. <br /> Failure to provide a written report to this office now places you <br /> liable for civil penalties under Article 25299 under Chapter6 .7 . <br /> JI <br /> A work plan that includes methods of determining the lateral and <br /> vertical extent of contamination along with proposed remediation <br /> of soil and or' groundwater that may be impacted is due in our <br /> office within. thirty (30) days prior to any site work. <br /> Fees for review activities and inspections associated with <br /> contaminated site assessment activities will not be billed by <br /> this office. The State Water Resource Control Board (SWRCB) will <br /> invoice you directly for activities performed under the auspices <br /> of the Local UST Oversight Program. The goal of the site <br /> evaluation performed by Public Health Services Environmental. � <br /> Health Division is to certify closures of underground tank <br /> leakage sites. Closure certification occurs only after <br /> mitigation activities are completed. <br /> A Letter of Acknowledgment is enclosed for your completion and <br /> return. Your acknowledgment of the placement of your site into <br /> _ -.the Local UST Oversight Program as stated above is respectfully <br /> requested.
