Complete this form for any <br /> drainage of secondary containment. This report must be kept on file by the designated person accountable for spill <br /> prevention for each tank and/or facility for at least three years. <br /> Description:Discharge associated with emptying the secondary containment of UNCONTAMINATED storm water. <br /> Location: Bulk Storage Area/Underground Piping Sump/Loading Rack <br /> (circle one) <br /> Inspection performed by: Title: <br /> Date: <br /> Drain valve initially in locked/closed position? YES / NO <br /> (circle your answer) <br /> Does the secondary containment have storm water build up?
