Eight Mile Rd. <br /> Stockton, CA 95209 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) has reviewed the <br /> "Third Quarter 1992 Groundwater Monitoring Report" as submitted by RESNA dated October 26, 1992 <br /> received on October 30, 1992 and has the following comments. <br /> The quarterly report for this site has not been submitted to PHS/EHD for "First Quarter 1992" as directed <br /> in correspondence to you on August 6, 1992. In addition, reports have been late for quarters that have <br /> been submitted. <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California Code of <br /> Regulations, Title 23, Chapter 16) a quarterly monitoring report shall be submitted to Public Health <br /> Services, Environmental Health Division and Central Valley Regional Water Quality Control Board <br /> (CVRWQCB) until the investigation and cleanup of the site is deemed adequate. The next quarterly <br /> report should be submitted by January 15, 1993. All quarterly monitoring reports are to show the: <br /> -status of investigation and cleanup activities including the results of all <br /> investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the <br /> investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and any <br /> contaminated soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and changes in the above <br /> measurements over the last quarter. <br /> PHS/EHD hereby directs you to continue monitoring and sampling all monitoring wells on site and to <br /> submit reports by the 15th of the month following the quarters end. After a complete annual quarterly <br /> monitoring series has been received on time and all information requested has been included, PHS/EHD <br /> will evaluate your request for modifying sampling events of these existing wells. <br /> Failure to submit reports or comply with regulatory directives may result in "UST Cleanup Fund" <br /> ineligibility and/or a request for formal enforcement action. <br />� A Division of San )mgwn County Health Care Services <br /> �� <br />
