SWRCB-CUF#: 003668 Stockton , CA . 95219 Global ID# : T0607700310 In correspondence dated July 9 , 2007 , the San Joaquin County Environmental Health Department (SJCEHD ) approved the Addendum to Work Plan for Vertical and Lateral Subsurface Investigation. This work plan was required by the SJCEHD in correspondence dated January 2 , 2007 . As of the date of this letter, the work plan has not been enacted and the SJCEHD Well Permit Application Forms and fees have not been submitted . The SJCEHD has not received any update on the status of the pending field work and will not allow the investigation to be held up any longer. You are hereby directed to submit a Well Permit Application Form and $ 383 per parcel to the SJCEHD by November 7 , 2007 . You are hereby directed to enact the approved work plan and install the monitoring wells before November 30 , 2007 . You are hereby directed to submit the report of findings for this phase to Geotracker so that it is available for the SJCEHD to review no later than January 30 , 2008 . The report is to be in the format of an updated Site Conceptual Model . Please make arrangements with your C-57 drilling contractor immediately and advise the SJCEHD of the date the drilling will commence . The permit copies will be forwarded to you when an appointment is confirmed by the SJCEHD staff.
