Highway 12 Lodi, CA 95242 Cj :Ji PRE-APPROVAL OF CORRECTIVE ACTION COSTS, CLAIM NO. 003668, SITE ADDRESS: 11530 8 MILE RD W, STOCKTON, CA 95219 I have reviewed your request, received on June 20, 2000, for pre-approval of corrective action costs. The total cost pre-approved as eligible for reimbursement for performing one event of groundwater monitoring and sampling and installation of one monitoring well is $7,891 . Please note that this pre-approval is valid only if the San Joaquin County EHD (County) approved the proposed work. Be aware that this pre-approval does not constitute a decision on reimbursement: all reasonable and necessary corrective action costs for work directed and approved by the County will be eligible for reimbursement per the terms ofyour Letter of Commitment at costs consistent with those pre-approved in this letter. All future costs for corrective action must be approved in writing by Fund staff. Future costs for corrective action must meet the requirements of Article 11, Chapter 16, Underground Storage Tank Regulations. It is my opinion that it is unnecessary to obtain three bids for this scope of work; the Fund's three bid requirement is waived for this scope of work. Please remember that it is still necessary to submit the actual costs of the work as explained in the Reimbursement Request Instructions to confirm that the costs are consistent with this pre-approval before you will be reimbursed. Please insure that your consultant prepares their invoices to include the required break down of costs on a time and materials basis, that invoiced tasks are consistent with the original proposal, and that reasonable explanations are provided for any changes made in the scope of work or increases in the costs. When the invoices are submitted you must include copies ofall: • subcontractor invoices, • technical reports, and • applicable correspondence from the County.
