ATTACHMENT A • • -3- <br /> ORDER NO, 5-00-119 <br /> APPLICATION REQUREMENTS <br /> GENERAL ORDER FOR DISCHARGE OF <br /> GROUND WATER FROM CLEANUP OF <br /> PETROLEUM FUEL POLLUTION <br /> TO SURFACE WATERS <br /> D. Public Notice Requirements <br /> 1. The Applicant shall submit to the Board a list of names and mailing addresses of nearby <br /> residents,including all adjacent property owners, and all residents within a 500 foot radius . <br /> of the treatment system and discharge point. <br /> 2. Discharges to publicly owned and operated storm water collection and conveyance systems <br /> must have written approval (submitted with the application) of the public agency that <br /> owned or operates the facility. <br /> 3. The applicant shall send letters, in an approved format, to each of the above interested <br /> parties listed in 1 and 2 above,the local County Health Department.and the California <br /> Department of Fish and Game describing the proposed project and including the'following <br /> information: <br /> a)• Describe the cleanup project and the involved chemicals of concern, <br /> b) Location of treatment system and discharge(both narrative and on map), <br /> c) Explain permit application and project implementation time schedule, <br /> d) Describe permit discharge limits and monitoring program, <br /> e) State in letter that interested parties have two weeks from date of letter <br /> to submit comments to Regional Board office, <br /> 4.. The Applicant shall submit a copy of the"Public Notification Letter" described in 3 above <br /> and shall submit'a certification on who was sent a copy of the letter. <br /> 16 June 2000 <br /> GKVflm <br />
