ORDER NO. 5-00-119 -7- <br /> INFORMATION SHEET <br /> GENERAL ORDER FOR DISCHARGE <br /> TO SURFACE WATERS <br /> OF GROUND WATER FROM CLEANUP OF <br /> PETROLEUM FUEL POLLUTION <br /> ATTACHMENT.0 <br /> NPDES Application Forms.(i and 2D), a Report of Waste.Discharge, and a filing fee will be <br /> submitted by each proposed Discharger,providing information on the discharge as detailed in <br /> Attachment A. If a proposed discharge does not qualify for this general permit,the Regional <br /> Board may consider it for adoption as an individual permit. If the Executive Officer finds that <br /> the proposed discharge qualifies for coverage under this General Order, the Discharger will be <br /> issued a Notification of Applicability. If significant public comments are received during the <br /> 15 day public commenting.period, described in Attachment A,the proposed discharge may be <br /> considered for an individual permit or for coverage under this General Order at a meeting of the <br /> Regional Board. The public commenting period is generally limited to 15 days upon notice of the <br /> Discharger's proposed action. Those actions and public notification procedures are described in <br /> the application process,contained in Attachment A. <br /> Upon completion of treatment and cessation of the discharge, the Discharge shall request official <br /> termination of coverage under this General Order from the Executive Officer. Failure to do so <br /> will result in the Discharger's continuing responsibility for payment of the annual fee associated <br /> with the General Order. . <br /> 16 June 2000 <br /> GKV/lm <br />
