For a municipality,state,federal or other public agency:by either a principal executive <br /> officer or ranking elected or appointed official. <br /> d. A duly authorized representative of a person designated in 6a, 6b or 6c of this requirement <br /> if: <br /> (1) the authorization is made in writing by a person described in 6a, 6b, or 6c of this <br /> provision, <br /> (2) the authorization specifies either an individual or a position having responsibility for <br /> the overall operation of the regulated facility or activity,such as the position of plant <br /> manager, superintendent, or position of equivalent responsibility. (A duly authorized <br /> representative may thus be either a named individual or any individual occupying a <br /> named position), and <br /> (3) the written authorization is submitted to the Board. <br /> Each person signing a report required by this Order or other information requested by the Board shall <br /> make the following certification: <br /> "I certify under penalty of law that this document and all attachments were prepared under my <br /> direction or supervision in accordance with a system designed to assure that qualified personnel <br /> properly gather and evaluate the information submitted.Based on my inquiry of the person or <br /> persons who manage the system, or those persons directly responsible for gathering the <br /> information, the information submitted is, to the best of my knowledge and belief, true, accurate, <br /> and complete. I am aware that there are significantpenaltics for submitting false information, <br /> including the possibility offine and imprisonment for knowing violations." <br /> The Discharger shall mail a copy of each monitoring report and any other reports required by this <br /> Order to: <br /> Central Valley Regional Water Quality Control Board <br /> 11020 Sun Center Drive,#200 <br /> Rancho Cordova,CA 95670-6114 <br /> In addition, dischargers designated as a"major"discharger shall transmit a copy of all monitoring <br /> reports to U.S.EPA(see address in Provision G. 10). <br /> E. DEFINITIONS: <br /> 1. The daily discharge rate is obtained from the following calculation for any calendar day: <br /> N <br /> Daily discharge rate(lbs/day)=8.34 E Qi C; <br /> N I <br /> In which N is the number of samples analyzed in a day. Q;and Ci are the flow rate(mgd)and the <br /> constituent concentration(mg/1),respectively,which are associated with each of the N grab samples <br /> that may be taken in a day.If a composite sample is taken, Ci is the concentration measured in the <br /> composite sample and Qi is the average flow rate occurring during the period over which samples are <br /> composited. <br />
