The Report is not a monitoring report, but a submittal of information <br /> accompanying Form 200 Report of Waste Discharge (RWD). <br /> The RWD is not complete because Environmental Protection Agency Forms 1 and 2D were not <br /> submitted, and the application fee submitted was incorrect. Regional Board staff made an error <br /> when we told you the fee was $1,800. The application fee for the General Order for Discharge <br /> to Surface Waters of Groundwater from Cleanup of Petroleum Fuel Pollution (General Order <br /> No. 5-00-119) is $5,688, in accordance with State Water Resources Control Board Resolution <br /> No. 2003-0064. Regional Board staff in the Site Cleanup Unit were unaware that the threat and <br /> complexity categories had been assigned to General Order No. 5-00-119 and that an 18.5 percent <br /> National Pollutant Discharge Elimination System surcharge applied. An additional amount of <br /> $3,688 is needed along with Forms 1 and 2D to complete the RWD. We apologize for this <br /> inconvenience. Please provide the remaining amount of the application fee and the forms by <br /> 1 June 2004. <br /> We have enclosed the General Order No. 5-00-119,Form 1, Form 2D, and the Standard <br /> Provisions and Reporting Requirements (Standard Provisions). Please review the Standard <br /> Provisions for monitoring, reporting, and signatory requirements. <br /> In our 21 January 2004 letter, we concurred with the request for an extension for submittal of the <br /> Site Assessment Report and stated the new due date was 17 February 2004. It is now more than <br /> two months beyond the extended deadline and we still have not received this report. If you do <br /> not submit the Site Assessment Report by 14 May 2004, we will recommend the Executive <br /> Officer issue a Cleanup and Abatement Order with enforceable time schedules for site <br /> investigation and cleanup. <br /> California Environmental Protection Agency <br /> 0 Recycled Paper <br />
