Eighth St. <br /> Stockton, Ca. <br /> The results from a recent environmental assessment indicate the presence of contaminants at the <br /> above site. This is to notify you that San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) has filed a Contaminated Site Report which places this site on our local <br /> Contaminated Site List. <br /> Should you wish to further investigate the contamination at this site, or desire comments on the <br /> previous investigation, contact Central Valley Regional Water Quality Control Board, Philip Isorena <br /> (916) 255-3049. Contamination originating from above ground sources is referred to the regional <br /> board. <br /> Fees for review activities and inspections associated with contaminated site assessment activities will <br /> be billed by this office. The site is being removed from the Local Oversight Program so you will not be <br /> billed by the State Water Resources Control Board as you were previously notified in a PHS/EHD letter <br /> dated August 8, 1993. PHS/EHD will invoice you directly for activities performed under the auspices <br /> of the San Joaquin County Board of Supervisors Resolution #90-1828. <br /> Please sign the enclosed Billing and Acknowledgement form where indicated, and return to San <br /> Joaquin County Environmental Health Department. <br /> If you have any questions regarding this matter contact Michael Collins, of our staff at (209) <br /> 468-0338. <br /> rnest M.
