BASELINE "
AUG s 2000
ENVIRONMENT HEALTH
PERMIT/SERVICES
MEMORANDUM
Date: 7 August 2000 Job No.:Y0310-00
To: Michael Infuma, San Joaquin County Public Health Services, Environmental Health
Division(PHS-EHD)
From: Rhodora Del Rosario, BASELINE Environmental Consultin
Subject: Request for Boring Permit for the Weber Block Property, tockton, California
As we discussed previously, we are requesting a second boring permit from the PHS-EHD. The first
boring permit that was issued in 1998 is expired. Our scope of work is limited to the installation of one
hydropunch to collect one grab groundwater sample at the Weber Block property, which is bounded by
Channel,El Dorado, and Center Streets and Weber Avenue in Stockton,California. We are scheduled to
install the hydropunch on Thursday, 17 August,2000,pending the receipt of the permit from PHS-EHD.
We have enclosed,as you requested, permit application materials, documentation that has been completed
for the Weber Block Property, and a permit fee of$89. Specifically, enclosed are: 1) completed well
permit application; 2)licensed contractor declaration; 3) Scope of Work letter dated 26 July 2000; 4) 24
July 2000 Memorandum from BASELINE to the Department of Toxic Substances Control(lead agency)
regarding the grab groundwater sample collection; and 5) the March 2000 Revised Preliminary
Endangerment Assessment prepared for the site. Based on your discussion with Bill Scott of our office,
the MFR form that was completed for the first permit(issued in 1998)will be used for this permit,so we
have not enclosed a new MFR form; the existing MFR form indicates that labor costs would be billed
directly to the City of Stockton.
I look forward to discussing this request for boring permit with you before 17 August 2000. Please contact
me or Bill Scott at(510) 420-8686. Thank you.
