(AGE), on your behalf for the above-referenced site. The
EHD had directed the submittal of a well destruction work plan by letter dated 29 March 2013.
In the work plan, AGE proposes to destroy monitoring wells MW-5, MW-6, MW-9 and MW-10 by
pressure grouting; and MW-1A/B, MW-2A/B, MW-3A/B, MW-4, MW-7 and MW-8 by removing
the well components by over-drilling the wells.
The EHD approves the work plan with these conditions:
• When pressure grouting the wells, apply 25 pounds per square inch (psi) of liquid grout
pressure for a minimum of ten minutes;
• When drilling out nested wells MW-1A/B, MW-2A/B and MW-3A/B, use augers larger
than the proposed twelve-inch augers and center the augers on the deepest casing.
The diameter of the augers must be large enough to remove all well casings, screens,
filter packs, and grout sealant installed during construction of the wells.
With these conditions the EHD approves the work plan to destroy the wells at this site. Please
submit a copy of the encroachment permit issued by the City of Stockton, the Well and Boring
Permit Application for the well destruction, Master File Record form and appropriate fees for
each parcel. The permit fee for the destruction of each well is $125.00, along with a $375.00
Inspection Activity fee per parcel. Since nested wells are counted as one well, and all wells in
the City of Stockton right-of-way are counted as residing on one parcel, a fee of $2000.00 (10
wells x $125.00 + $375.00 x 2) will be required for the well destruction permits.
Please provide the EHD certification in writing that all record owners of fee title to the above-
referenced site have been notified of your request for closure. This is in accordance with
