(AGE), on your behalf for the above-referenced site. The <br /> EHD had directed the submittal of a well destruction work plan by letter dated 29 March 2013. <br /> In the work plan, AGE proposes to destroy monitoring wells MW-5, MW-6, MW-9 and MW-10 by <br /> pressure grouting; and MW-1A/B, MW-2A/B, MW-3A/B, MW-4, MW-7 and MW-8 by removing <br /> the well components by over-drilling the wells. <br /> The EHD approves the work plan with these conditions: <br /> • When pressure grouting the wells, apply 25 pounds per square inch (psi) of liquid grout <br /> pressure for a minimum of ten minutes; <br /> • When drilling out nested wells MW-1A/B, MW-2A/B and MW-3A/B, use augers larger <br /> than the proposed twelve-inch augers and center the augers on the deepest casing. <br /> The diameter of the augers must be large enough to remove all well casings, screens, <br /> filter packs, and grout sealant installed during construction of the wells. <br /> With these conditions the EHD approves the work plan to destroy the wells at this site. Please <br /> submit a copy of the encroachment permit issued by the City of Stockton, the Well and Boring <br /> Permit Application for the well destruction, Master File Record form and appropriate fees for <br /> each parcel. The permit fee for the destruction of each well is $125.00, along with a $375.00 <br /> Inspection Activity fee per parcel. Since nested wells are counted as one well, and all wells in <br /> the City of Stockton right-of-way are counted as residing on one parcel, a fee of $2000.00 (10 <br /> wells x $125.00 + $375.00 x 2) will be required for the well destruction permits. <br /> Please provide the EHD certification in writing that all record owners of fee title to the above- <br /> referenced site have been notified of your request for closure.
