The top three feet or more of the well shall be drilled out with an <br /> auger that is at least one foot larger in diameter than the auger originally utilized to install <br /> the well, and the soil around the cut-off casing removed to a depth of an additional six <br /> inches below the new `top' of the cut-off casing. The grout sealant shall spill out of the <br /> top of the well casing to form a mushroom cap seal, with at least 12 inches of neat <br /> cement grout poured 6 inches above and 6 inches below the top of the cut-off casing. <br /> • EHD records indicate the presence of `nested' wells installed using ten-inch augers. The <br /> EHD has found destruction results of nested wells are best obtained when an auger <br /> larger than the original auger is used and centered on the deepest casing (which is off- <br /> center in the original borehole). The augers need to be sized sufficiently to reach out <br /> and remove all well casings, screens, filter packs, and grout sealant installed during <br /> construction. <br /> • All groundwater displaced to the surface during grout installation shall be pumped out of <br /> the excavation to allow for the EHD inspection and verification that the well has been <br /> properly destroyed. Contaminated groundwater pumped out of the excavation shall be <br /> containerized for proper disposal. <br /> • This site has three wells located off-site in the City of Stockton right-of-way. Please <br /> submit a copy of a valid encroachment permit when submitting the EHD Well and Boring <br /> Permit Application form for these wells. <br /> After the EHD has approved the well destruction scope of work proposal, you may submit an EHD <br /> Well and Boring Permit Application form, Master File Record form, Encroachment Permit issued <br /> by the City of Stockton, and the appropriate fees for each parcel. The permit fee for the <br /> destruction of each well is $125.00, along with a $375.00 Inspection Activity fee per parcel. A <br /> review of the site records indicates that 13 wells currently are installed. A fee of $2,000.00 (10 <br /> wells x $125.00 + $375.00 x 2) will be required for the well destruction permits. (FYI — To <br /> determine the fees for nested wells, each pair of nested wells is considered as one well for <br /> permitting and fee purposes.) <br /> Please provide the EHD certification in writing that all record owners of fee title to the above- <br /> referenced site have been notified of your request for closure. This is in accordance with Section <br /> 25297.15 of the California Health and Safety Code. Submit a complete list of record owners of <br /> fee title and their mailing addresses to the EHD. If you are the sole landowner, provide a <br /> statement to this effect to the EHD.
