QST ENVIRONMENTAL <br /> CONTINGENCY PLAN <br />' Fire or Explosion Response: <br /> In the event of a fire or explosion, the area will be immediately evacuated and the Fire Department will be summoned <br /> as soon as possible If at all possible, workers in the exclusion zone will exit through the contamination reduction zone <br /> and will, at a minimum, remove or scrub their outer boots and remove their outer layer of protective clothing prior to <br /> proceeding to the assembly location. Site personnel will gather at a designated location upwind of the fere/explosion (use <br />' predominant wend direction) The location shall be established during the daily safety meetmng, and a head count well <br /> be taken at the location. Upon their arrival, notify the fire department of the location and nature of the fire/explosion <br /> AIso provide information on the location and identification of hazardous and flammable materials on site <br /> If it can be done safely, site personnel who have had the appropriate training may perform the following <br /> • Use available on-site fire extinguisher to control or extinguish the fire if it is small and localized, <br /> • Remove or isolate flammable or other hazardous materials that may contribute to the fire, <br /> • Begin containment and recovery of any spilled materials (see below) <br /> Spill Response: <br />' In the event of a spall, the SHSO shall be notified immediately Procedures for exposure monitoring and control as <br /> �th,"dined this HASP shall be followed, including upgrading the LOP for spell containment/clean-up if necessary FTMs <br /> m ould stop the spill source and contain and cleanup the spell as necessary and appropriate After the cleanup is <br /> completed, air monitoring well be conducted by the SHSO to ensure that airborne levels of the contaminant(s) are at a <br /> safe or appropriate level <br /> ' Spill Reporting Procedures: Any spill should reported to the QST office and the appropriate agencies as soon as <br /> possible <br /> For a Medical Emergency: <br /> ' If trained and willing, initiate first aid and get medical attention for the injured person(s) immediately Have the injured <br /> person(s) transported to the nearest medical facility (see above) or call ambulance as necessary As soon as possible, <br /> notify the mjured person's supervisor or the project manager Supervisors/PM's notify your LHSR and, in the event of <br /> a chemical or bloodborne pathogens exposure, EMR immediately <br /> ' Emergency Decon Procedures <br /> The level of decon in a medical emergency well be determined by the extent of the injury For minor injuries, personnel <br /> must go through the proper decontamination sequence as stated in this HASP <br /> In life-threatening emergencies or when decontamination may aggravate the condition, decontamination procedures may <br /> omitted If decon cannot be performed, a FTM should accompany the injured worker to the medical facility, if <br /> sible, to provide information to medical response personnel regarding the contaminants and decon procedures In lieu <br /> ' of decontamination, actions such as removal of the outer layer of protective clothing or wrapping the victim in plastic <br /> ' BPS Property No 127434 Stockton-6498090-September 23 1998 <br />
