AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FERGUSON ENTERPRISES 530 PORT OF STOCKTON EXPY, STOCKTON June 14, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> At least 115 gallons of cleaner with sodium hydroxide, 123 gallons of bleach (sodium hypochlorite), and 72 gallons <br /> of sulfuric acid were found on site that have not been reported. Any material that meets or exceeds the reportable <br /> quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting <br /> System(CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to <br /> the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show/address loading areas, internal roads, and emergency shutoffs. A site map shall contain a <br /> north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 8 HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program was lacking or insufficient. The training program did not address the Immediate notification <br /> contacts to the appropriate local emergency response personnel and to the unified program agency. Training <br /> programs for all employees and annual training, including refresher courses, shall include safety procedures for the <br /> event of a release or threatened release of a hazardous material, including, but not limited to, familiarity with the <br /> facility's emergency response plans and procedures. These training programs may take into consideration of each <br /> employee. Immediately prepare or update the facility's training program, log into the California Environmental <br /> Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the <br /> EHD for approval. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually for 2019. The hazardous materials inventory shall be submitted by January 15 of each <br /> calendar year and may be submitted beginning November 1 of the previous year.
