The training program was lacking or insufficient. The training program did not address the immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency ( spill reporting requirements ) . Training programs for all employees and annual training , including refresher courses , shall include safety procedures for the event of a release or threatened release of a hazardous material , including , but not limited to , familiarity with the facility ' s emergency response plans and procedures. These training programs may take into consideration of each employee . Immediately prepare or update the facility ' s training program , log into the California Environmental Reporting System ( CERS) at http : //cers. calepa . ca , gov/ , upload the correct or updated information , and submit to the EHD for approval . This was corrected on site . This is a Class II violation. 9 HSC 25508 ( a) ( 1 ) Failed to annually review and certify that the business plan is complete , accurate , and up to date . The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System ( CERS) annually for 2018 and 2019 . The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year . Immediately log into the CERS at http : //cers . calepa . ca . gov/, enter the correct or updated information , and submit to the EHD for approval . This is a Class II violation. 10 HSC 25508 . 1 (a ) - (f) Failed to submit revisions to business plan within 30 days of substantial change in operations . During the inspection two 115 cubic foot cylinders of compressed oxygen were found and the updated information has not been submitted . Within 30 days of one the following events , business shall electronically update the information submitted to the California Environmental Reporting System ( CERS) : - A 100 percent or more increase in the quantity of a previously disclosed material . - Any handling of a previously undisclosed hazardous materials - A change of business address , business ownership , or business name . Immediately log into the California Environmental Reporting System ( CERS) at http : //cers . calepa . ca . gov/, enter the correct or updated information , and submit to the EHD for approval . This was corrected on site . This is a Class II violation. 11 HSC 25505 ( a) (4 ) Failed to provide or document initial and/or refresher training to appropriate personnel . Employee training records were not available at the time of inspection . The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training . This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years . Immediately provide employee training for appropriate personnel and submit a copy of the training records to the EHD .
