Any change of information on these forms must <br /> be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the <br /> facility and tank forms to the EHD. Complete a separate tank form for each tank. <br /> #4 Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility immediately. <br /> #5 & 6 Current approved copies of the monitoring and response plans were not found on site. The <br /> monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br /> of the response plan for approval by the EHD by 11-27-10. An approved copy will be returned for your <br /> records. <br /> #7 Maintenance records for the 9-3-10 diesel stp sensor alarm, 2-18-10 diesel stp, 87A stp, 91 stp, 87B <br /> stp, diesel/87 annular, and 91/87 annular sensor alarms were not found on site. These records shall be <br /> kept on site for at least 3 years . Immediately locate and maintain the maintenance records for the <br /> 9-3-10 diesel stp sensor alarm, 2-18-10 diesel stp, 87A stp, 91 stp, 87B stp, diesel/87 annular, and <br /> 91/87 annular sensor alarms on site and submit copies to the EHD by 11-27-10. <br /> #11 The 87A stp and diesel stp sump sensor was raised approximately 5 inches off the lowest point of <br /> the sump almost 1/4 up from the bottom of the saddle and not located to detect a leak at the earliest <br /> opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br /> opportunity. Technician corrected this during the inspection by repositioning the sensor to the lowest <br /> part of the stp sump. <br /> #13 The Bravo box float and chain in the north diesel main udc failed to stop the flow of product at the <br /> dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> adjusted the chain and verifies functionality. <br /> #21 Liquid was observed in the south diesel satellite udc. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector Received Bqy� Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
