Lodi, CA 95240 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace the existing diesel dispenser with a new Gilbarco Legacy <br />Series model #JHA -100 Dual Master Dispenser and a Bravo conversion frame CONV-62000 on <br />the underground storage tank (UST) system at the above noted address. The permit has been <br />approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Primary piping soap test: One hour pneumatic pressure test at 110% of operating <br />pressure, or 40 psi, whichever is greater for the diesel line. <br />b. Inspector to verify that the Bravo Conversion Frame CONY -62000 has been installed <br />correctly with the Gilbarco Legacy Series model #JHA -100 Dual Master Dispenser. <br />Please be aware that the facility cannot operate the UST system or receive fuel in <br />the diesel tank prior to the EHD inspection and approval. <br />4. If concrete will be broken near the diesel piping, a secondary containment test must be <br />performed on the diesel piping once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />
