Terms and Conditions: 1)"RCRA"Land Disposal Restriction Notification'must accompany each manifest.2) Liquid shipments will be assessed a rinse charge of$200.00 per tanker and$35.00 per tote. Additional charges will apply if a washout is required. 3)Demurrage charges of$85.00 per hour will apply to loads that require in excess of 2 hours loading or unloading time. 4)Liquid Totes are billed a usage fee of$60.00 each.5)Solid tub skids are billed a usage fee of$60.00/each. Approval and Acceptance: The undersigned agrees and certifies that he/she is authorized to act on behalf of Customer, and Customer guarantees by his/her signature to compensate 21st Century EMI Inc.d/b/a PSC Environmental Services in full for all actual services performed as described above,which are due and payable 30 days after receipt of the invoice. All past due accounts shall accrue interest at the rate of 1.5%per month. No verbal estimates are valid or enforceable.
