<br /> This letter report summarizes ft July 20, 1991,installation of an off-site monitoring well(MW-4),
<br /> aPpraxlmalely 90 feet downgradlent of the above-referenced site. Three monitoting wdfs are located nn
<br /> the she(MW-1. MVI-2.and MW-3). Petrdeum hydrocarbons have been detected in groundvrater
<br /> sample~rollacled From two of these monitoring wells,MW-1 and MWti3(Figure 1). Monitor!ng well MW-
<br /> 4 was Installed to provJdc InformEtlon on groundwater condhiOns downgradlent of the site.
<br /> Instsl'ation of monhoring wall MWA was pertarmed under Chevron U.S.A. Int.Release Nrx 574-2990.
<br /> The scape of work Included obtaining the necessary permfts prior to w011 Install&tlorr, colleciing sol!
<br /> samples from the soli boring made prior to the Installation of the monitoring well,ano Installing and
<br /> developing this monitoring w0. A report detalllrg the rosufts of groundwater monitoring and sampling
<br /> was Issued separately.
<br /> Three permits wore obtained from regulatory bodies prior to the Installation of this monitoring well. A
<br /> lnonhoring welt Installation pefmft was obtained from the San Joaquin County Public F:ealth Services
<br /> Department Environmental Health DIvIslon(S,ICEHE));this permit requ!red that groundwater be sampled
<br /> for fecal coliform because the monitoring well Is located within 50 feet of a City of Stockton sewer line.
<br /> Two permits were obtained from the City of Stockton Public Works Department,because the locatlon of
<br /> monitoring well MW-0 Is on a public right-.4-way(South El Dorado Street):a revocable 4ennit and an
<br /> encroachment permit to perform work on a public right-of-way. Copies of all rel&mrtt permits are
<br /> Included In Attachment A
