Unless the EHD informs you otherwise, you may proceed with the well destructions <br /> if in compliance with the conditions noted above. <br /> Please &ubmit an EHD Well and Boring Permit Application form along with a Master File Record <br /> form, encroachment permit issued by the City of Manteca and appropriate fees for each parcel. <br /> The pennit fee for the destruction of each well is $125, along with a $375 Inspection Activity fee <br /> per parcel. A review of the site records indicate that ten (not eight as previously indicated by <br /> letter dated 24 December 2013) monitoring wells, three oxygen-injection sparge wells and five <br /> soil gas probes currently are installed. A fee of $3,000.00 (18 x $125 + $375 x 2) will be <br /> required for the well destruction permits.
