Environmental Health Division (SJCEHD) for approval These modifications are made as requested in the telephone conversation with Mrs Carol Oz of the SJCEHD on February 22, 1999 The proposed sod boring located at the southern section of the site on the Work Plan map will be relocated to the area south of monitoring well MW-2 (Figure 2) Monitoring well MW-5 which was proposed to be a dual-nested well will not be nested Instead a single well will be installed in this bonng with the screened interval to be from the depth of boring to approximately five feet above the bottom of the boring A site-specific Health and Safety Plan (HASP) has been prepared by IT Corporation as required by the Occupational Safety and Health Administration (OSHA) Standard "Hazardous Waste Operations and Emergency Response"guidelines (29 CFR 1910 120) The HASP contains site-specific health and safety information such as material safety data sheets, air monitoring action levels, emergency procedures and phone numbers, hazard analysis, directions to the nearest hospital with an emergency facility(Figure 3) and other health and safety information required by OSHA guidelines The document will be reviewed and signed) by all IT Corporation personnel and subcontractors performing work at the site Within four to six weeks of receipt of all field and analytical findings, IT Corporation will prepare a report summarizing the data collected under the scope of work detailed above The report will document the methods and results of the work, summarize laboratory analytical results, and include appropriate maps
