E1 Dorado St. SITE CODE: 2557 <br /> Stockton, CA <br /> San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) has recently reviewed the project chronology <br /> of the above mentioned site from the letter addressed to <br /> Mr. Scott Malm, Attorney At Law, dated July 27 , 1992 . The report <br /> states that hand auger borings were drilled during the June 22 , <br /> 1992 sampling. <br /> Pursuant to the Code of San Joaquin County, Section 5--4000 et <br /> seq. , a permit shall be secured for all test holes or soil <br /> borings. Work at the above mentioned site was completed without <br /> permit or guidance from PHS/EHD. Therefore, enclosed is a permit <br /> application to be completed by you and returned to this <br /> department along with the permit and penalty fees totalling <br /> $178 . 00. of primary concern to this department is the <br /> destruction of soil borings at this site, documentation must be <br /> submitted stating how the holes were destroyed. <br /> Prior to any future work being done at this site, 48 hours <br /> advance notice must be given and any necessary permits must be <br /> obtained from this department. <br /> Submit the application, fees, and description of the boring <br /> destruction to this department by August 28 , 1992 . <br /> If there are any questions, contact Michael Collins, EHS, at <br /> (209) 468-0338 . <br /> Jogi Khan , M.D. , M.P.H. <br /> Health icer . <br /> i <br /> La�urieF . 0 Ea, RENS, Program Manager <br /> nvironmental Health Division <br /> LAC/MC: lb <br /> A Division of San Joaquin County Health tare Services �� <br />
