Financial responsibility documents (Certificate of Financial Responsibility and CFO letter) have not <br />been submitted to the San Joaquin County Environmental Health Dept (EHD). Current financial <br />responsibility documents are required to be submitted annually. Submit these documents to the EHD by <br />4-18-13. <br />106. The UST response plan on site is not current and not approved by the EHD. The response plan <br />must be current and approved by the EHD. Complete and submit a copy of the response plan for <br />approval by the EHD by 4-18-13. An approved copy will be returned for your records. <br />113. Designated operator Luis Torres conducted the monthly designated operator inspection on 1-14-13. <br />The EHD was not notified of the change in designated operator within 30 days of the change. Send the <br />change of designated operator documentation to the EHD by 4-18-13. <br />201. The 3-21-2012 secondary containment test results (Alltech Petro) were not found on site. These <br />records shall be kept on site for at least 3 years. Immediately locate and maintain the the test results on <br />site and submit copies to the EHD by 4-18-13. <br />Notes <br />1. Complete the following by April 18, 2013 and send to the San Joaquin County Environmental Health <br />Department: <br />a. The Return to Compliance Certification form. <br />b. A statement explaining how each violation was corrected. <br />c. Supporting documentation. <br />2. Discussed hazardous waste regulation and the CERS program with the operator. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUB ECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
