You may now <br /> proceed with the well destructions as described and directed by the EHD by letter dated 21 <br /> May 2013. <br /> Submit the EHD Well and Boring Permit Applications, Licensed Contractors' Declaration <br /> (LCD), Master File Record (MFR), encroachment permit issued by the City of Stockton, and <br /> appropriate fees for each parcel. The permit fee for the destruction of each well is $125, <br /> along with a $375 Inspection Activity fee. A review of the site records indicates that fifteen <br /> monitoring and remediation wells currently are installed at this site. A fee of $2,625.00 ($125 <br /> x 15 + $375 x 2) will be required for the well destruction permits. (Continuous Multichannel <br /> Tubing (CMT) wells are considered as one well for permitting and fee purposes.)
